REGISTRATION INFORMATION

TO-DO BEFORE THE FIRST DAY OF TRYOUTS/PRACTICE:

• Register (Link to Registration)
• ​Upload your IHSA Sports Physical to your Snap (registration) account
  • All sports physicals must be uploaded to tryout/play (directions)
  • ​Keep a copy of the physical in your home files as well
    ​

PRO-TIP: Try to book your physical appointment in the summer each year, that way it will not expire mid-year or mid-season. Appointments fill quickly, so contact your healthcare provider to book an appointment right away. CVS, Walgreens, and other health clinics can also perform sports physicals.
